RPM: About
Remote Patient Monitoring (RPM) allows for active
monitoring and management of chronic conditions. RPM
utilizes digital technologies to securely collect patient data
and electronically transmit them to providers for
assessment and when needed, for immediate intervention.
RPM: Conditions
The chronic conditions that benefit from RPM include
hypertension, diabetes, congestive heart failure, and obesity
while acute conditions may consist of more fleeting or mild
illnesses such as asthma, nausea, or the common cold.
RPM: Eligibility
Medicare patients with acute and/or chronic conditions care can be enrolled in the program.
RPM: Devices
Types of common RPM devices:
- Blood Pressure Cuffs
- Glucometer
- Pulse Oximeter
- Wearables (Activity Trackers and Continuous Monitoring)
- Thermometer
- Scale (Weight)
RPM: Benefits
- With ACP, you gain a clinical team to aid you in closely watching your patient’s vital signs and other important measurements to know their real-time health status.
- You add an extra layer of care to your service from the wearables and devices we provide.
- These devices send an alarm when preset thresholds are crossed or alerts when measurements are not taken.
- Improves your patient engagement as they actively take their measurements and feel more accountable with the close attention they are receiving.
- Brings you billable non-face-to-face services that bring extra cash flow to your practice. You can bill for the first 20 minutes, an additional 20 minutes, and in supplying the equipment.
- RPM fulfills your core competency of providing value-based care that will improve your performance scores.
RPM: Get Started
- ACP just needs access to your EHR/EMR to assist you in identifying who is eligible to join.
- We can train your MAs and nurses and give them incentives to encourage patients to enroll.
- Once patients are identified, they will have to offer their consent that they are participating in the said program.
- We will then ship the devices to your patients and teach them about their proper use.
